About us

This service is for GPs and other referrers only.

This is the Single Point of Access (SPA) for healthcare professionals to use to refer adults with Richmond GPs to the following community-based services:

  • Intermediate Care Team
  • Community Physiotherapy
  • District Nurses including specialist services
  • Community matrons
  • Community Neuro-rehabilitation Team
  • Falls Service
  • Cardiac Rehab Service
  • Respiratory Care Team
  • Richmond Response and Rehabilitation Team
  • Desmond Diabetes
  • Speech and Language Therapy

The SPA team triage all referrals and ensure that they are directed to the appropriate service.

Referrals

All referrals need to be completed on the relevant Single Point of Access referral form.
