Appointments
We will make every effort to book appointments in advance at a time or setting convenient for you. This may be at home, in a clinic or at school. However, for home and school visits we are unable to give a specific time. If you would like a specific time slot, please book into our nurse led clinic.
Referrals
We accept referrals for babies, children and young people up to the age of 18, registered with a Richmond GP, with an identified nursing need. This may include acute episodes of care, such as intravenous antibiotics, or chronic/ complex health conditions, including palliative care.
We accept referrals from health care professionals, social care professionals, educational services, carers and families. We will require a referral form completed, however we welcome a telephone call to discuss if you are unsure or have any questions.
If we are unable to accommodate your referral, we will signpost you to the appropriate service.
Once a referral has been accepted, a nurse will undertake a full nursing assessment and agree a plan of care with the child/young person and their carers. We will make contact with family within 2 working days.
How to contact the team
Our phone lines are open Monday to Friday, 8am- 4pm. We have a voicemail service and always aim to return your call by the next working day.
We also welcome emails which are accessed throughout the day.
Please note that we are not an emergency service and for any urgent medical advice please contact your GP, 111 or 999 as appropriate.
