Who we are

The Children’s Community Nursing Service provides a range of services to children and young people who need health support in their home, clinic or community.

The team is made up of experienced children's (paediatric) nurses.

We work with children and young people of any age up to 18 years who are registered with a Richmond GP.

What we offer

We work with the child, their family or carers, and healthcare professionals. We will plan and deliver high quality, evidence-based, coordinated care.

As we do this, we will consider the needs of children and their families to make sure children are safe and well cared for.

We aim to reduce how often a child has to visit or stay in hospital or, if they do have to stay in hospital, how long they have to stay for.

Our appointments may be in our clinic, at home or at school.

Who we support

We care for children who need a wide range of nursing, including:

  • intravenous medication
  • injections
  • post-operative care
  • wound care
  • home oxygen therapy
  • vital signs monitoring, for example, blood pressure
  • central line care
  • cancer support, including regular blood sampling and line care
  • tube feeding (also called 'enteral feeding')
  • support for complex and life-limiting conditions
  • symptom management and end of life care.

This list does not cover everything we do. If your child has a nursing need that is not listed above, contact us and we will do our best to help.

We also provide support and training for:

  • managing eczema 
  • severe allergies and Epi-pen training for parents and nurseries
  • complex epilepsy, including teaching parents and nurseries how to give emergency medications
  • naso-gastric tube care
  • gastrostomy care
  • cancer support for schools.

Referrals

We accept referrals from healthcare professionals, parents and schools. However, the child must have an identified nursing need.

Call or email us to discuss a referral. We aim to make contact with the child, family or carer within 48 hours.

So that children can cared for in the community, we work with a range of teams, including:

  • GPs
  • paediatricians (doctors who specialise in looking after children)
  • hospital doctors
  • local and specialist centres
  • physiotherapists
  • dieticians
  • health visitors
  • speech and language therapists
  • school nurses
  • social workers
  • voluntary agencies.

How to contact the team

Our phone lines are generally covered 8am to 4pm, Monday to Friday. However, this does depend on how busy we are.

We are not an emergency service.

Sharing information

We will keep an accurate electronic record of your care. In some instances, we will also keep a paper record in your home.

Sometimes it is necessary to share your information with other health and social care professionals on your behalf. If you do not wish to have information about your child’s health needs shared, tell a member of the nursing team.

If a safeguarding incident occurs, we reserve the right to share information with partner agencies. This will only be done if it is strictly necessary.
